***Please arrive 30 minutes before your scheduled appointment time so that your pet can sit while their eyes are being dilated.  Please note that your pet’s eyes will remain dilated 4-6 hours after their exam and they may be sensitive to bright lights. Arriving less than 30 minutes in advance of your appointment could cause a delay in your appointment start time. ***

Payment Information

To finish booking your appointment, please follow the steps below to prepay for the initial consultation:

1. Please utilize the button below to prepay for the initial consultation. 
2. The button will launch a new screen. Click on the name of the location you will be visiting (San Rafael)
3. Read through the privacy policy, click on the box that you have read and accept terms & conditions, and click next. 
4. Completely fill out all boxes with an asterix (*). The invoice amount for an OFA exam is $100.
5. Click Submit.
6. Please complete this within 24 hours of speaking to an Animal Eye Care client service representative. If not received, your appointment will be removed from the appointment book.
7. If your appointment is within 24 hours, please pay the amount due by 5pm today.
